Please submit the following form to update your name on file with The Conference. Please note this process takes 3-5 business days to complete. If you are currently scheduled for an exam under an incorrect name, you will need to cancel your exam appointment and reschedule once your name has been updated. Official documentation is required to update a name on file with The Conference. You may upload the documentation using this form, OR email it to admin@theconferenceonline.org, OR fax it to 479-442-7090. Please Note: Request will not be processed unless application/order has been placed.
